A simple liquid treatment could change the way severe tooth decay is managed in young children.
Researchers have found that silver diamine fluoride (SDF), a liquid applied directly to a cavity, can stop the progression of tooth decay without drilling, injections, or anesthesia.
In a large randomized clinical trial published in JAMA Pediatrics in July 2026, the treatment successfully arrested significantly more cavities than a placebo among young children with severe early childhood caries.
The findings could be especially important for children who are too young to tolerate conventional dental procedures or who have extensive tooth decay requiring more complicated treatment.
🧪 What Is Silver Diamine Fluoride?
Silver diamine fluoride is a topical dental treatment containing silver and fluoride.
Instead of removing the decayed portion of a tooth with a dental drill, SDF is applied directly to the affected area. The silver component has antimicrobial properties, while fluoride helps strengthen and remineralize the damaged tooth structure.
The goal isn’t to rebuild the tooth or make the cavity disappear. Instead, the treatment aims to stop the decay from getting worse.
The procedure is minimally invasive and can be performed without drilling or local anesthetic.
👧 A Large Clinical Trial Included 830 Children
The new study enrolled 830 children between 1 and 6 years old who had severe early childhood caries.
The children received either 38% silver diamine fluoride or a placebo. Researchers then evaluated whether the cavities had become arrested at follow-up.
At six months, 54.0% of lesions treated with SDF were arrested, compared with only 22.5% in the placebo group.
At three months, the arrest rates were 57.5% with SDF versus 18.8% with placebo. At eight months, the rates were 50.2% and 17.4%, respectively.
These results demonstrate a substantial difference between the treatment and placebo groups.
